How Do I Request A Life Insurance Application?

What to Expect

When completing an application, expect to answer some basic personal background information. For example, in addition to name, address, phone number, and date-of-birth, expect the following personal questions:

• Your height & weight
• Personal lifestyle habits (i.e., smoking, drinking, exercise)
• Personal financial questions such as current and expected income

To avoid any issues throughout the process, it is important to be as accurate and honest as possible on the application. Typically, after the application IntelliQuote will arrange for a quick medical exam with a paramedical (a licensed healthcare professional contracted by the insurance company) at your home, office or a clinic selected by the insurance company. Once your application, medical exam and additional underwriting information have been reviewed, the company will either approve or deny your application for coverage. This process can take days or weeks, depending on if you have submitted a complete application, how long it takes to get medical information from your physician, and other underwriting information. Normal underwriting time for a decision on most life insurance applications is 4-6 weeks.
